Alera Healthcare provides two services at home:

  • Exercise physiology for tailored exercise programmes that build capacity over time, with clear progression and safety checks.

We do not offer occupational therapy or NDIS services. If physiotherapy or exercise physiology is not the right fit, we will say so early.

Continuity and a clear point of contact

Wherever possible, the same physio returns. Continuity helps seniors and carers know who is coming, what was practised last visit, and what to expect next. Coordinators and families keep a single clinical contact while funding paperwork stays with the registered Support at Home provider when that pathway applies.

Support at Home: associated, not registered

Under Support at Home, a registered provider holds the participant's funding and care plan. Alera Healthcare works as an associated provider, delivering physiotherapy and exercise physiology on behalf of those organisations.

We coordinate notes and next steps with the registered provider as authorised. Participant contributions and plan rules depend on the individual arrangement.

Other pathways we discuss on enquiry include DVA (where eligible) and private payment. Entitlements vary; we confirm expectations and fees before care begins.

When in-home physio is worth a conversation

Consider a short call if you or someone you support:

  • Has reduced confidence walking indoors or outdoors
  • Is returning home after hospital or rehab and needs follow-through in the house
  • Lives in residential aged care and needs Physio or EP input
  • Has a Support at Home plan and a registered provider ready to coordinate allied health
