GPs, hospital discharge planners, and Support at Home coordinators can refer a client for physiotherapy or exercise physiology at home. Use the form on this page. We aim to reply within one working day. Alera Healthcare is an associated Support at Home provider, working alongside registered providers, and is not a registered provider. We provide physiotherapy and exercise physiology, and we don't provide occupational therapy.

How GPs refer
Send the referral with the form on this page. Discharge planners and care coordinators use the same form. Care managers can also read what care managers need from a home physio partner. Families who are enquiring for themselves can book a 15-minute call instead.
What to include
- The client's name and a phone number or email we can use
- The suburb, so we can check Sydney Metro capacity
- The clinical priority in a sentence or two, such as mobility, balance, post hospital recovery, or a chronic condition
- The funding path if you know it: Support at Home, DVA, or paying privately
- Whether the question is physiotherapy, exercise physiology, or both
What happens next
