Refer a client for home physiotherapy or exercise physiology

Discharge planners and Support at Home coordinators can refer clients to Alera Healthcare for home physiotherapy and exercise physiology. We work as an associated Support at Home provider, not a registered provider. Physio and EP only. Prefer /referrals as the canonical referrer page (this URL shares the same destination).

What to expect

Alera Healthcare is an associated provider under Support at Home. We deliver clinical services on behalf of registered providers who hold the participant's funding and care plan, and we coordinate paperwork with them directly.

How to refer

    Send the referral

    Use the short /referrals form, call, or email. Include client name and clinical priority. Six fields only on the form.

  • 02

    We confirm suitability

    We respond within one working day, confirm Sydney coverage, and clarify Support at Home, DVA or private arrangements.

  • 03

    Matched clinician

    Wherever possible, the client sees the same physio or exercise physiologist.

  • 04

    Feedback loop

    We coordinate with the registered Support at Home provider as authorised. Clear notes, clear next steps.
